3. Enter the correct information about your operation then send to us email, fax, or mail.(Please send the appropriate fee or use the online system).
4. The forms may be completed with the fill in fields, or by hand. Please Fax or Mail signature pages.
Materials Forms (Substance)
1. Use the Brand Name Inputs Application if you are applying for review and approval of Substances or Brand Name Inputs which are intended for Organic Production. This appplication must be accompanied by a Brand Name Product Information Sheet for each product.
2. DO NOT USE these two forms for crop production information or processed foods.
